NON-IMMIGRANT VISA FORMS
All visa applicants having their visa application interview appointment date on or after March 1, 2010 must use the new electronic visa application form DS-160 and must present the DS-160 one page confirmation sheet with the bar code at the time of their interview appointment. All visa applicants having their visa application interview appointment date on or before February 26, 2010 must still present the completed and signed DS-156. Please note that all visa applicants are required to use the DS-160 as of March 1, 2010. All applicants failing to present the DS-160 electronic visa application form will be required to schedule a new appointment with the Visa Hotline. Please note that (for interview appointments on or before February 26, 2010) all applicants are still required to sue the visa application form (DS-156). The form MUST be completed on the PC, and printed out afterwards. All applicants failing to present the electronic visa application form or present it with broken barcode will be asked to schedule a new application appointment enabling them to return with the appropriate form. All information entered online is contained in a barcode - printed on the last page of the completed application form - which also needs to be presented at the time of the application.
